DO I QUALIFY?
If you’re a former patient who has not come in for an appointment to receive care in 3 months to just under 3 years. Patients who have not had an appointment in over 3 years will have be considered new patients.

BEFORE YOUR APPOINTMENT
EVALUATE YOUR HEALTH
We ask that if you are feeling ill or running a fever that you veer on the side of caution and reschedule your appointment when you’re feeling better.
UPON YOUR ARRIVAL
PATIENT ONLY
We ask that only the patient enter the building for your appointment. If you get a ride to your appointment we prefer the driver remain in the vehicle during the appointment, unless physical assistance is needed for mobility.
WEAR A MASK
Masks are required to be worn in our building and during your appointment.
CHECK IN
Please come in and check inn at the front desk first.
BRING YOUR ID AND UP TO DATE INSURANCE CARD
Our front desk team member will take copies of your up to date information.
FILL OUT NEW FORMS
As a returning patient with us, we will need you to fill out some paperwork once you’ve arrived. You will be required to fill out a few forms to help with assessing your current physical condition.
STAY 6’ APART
Our lobby has been reconfigured to reflect the 6’ distance needed to keep all Chiromed patients safe. Please do not move office furniture.
DURING YOUR APPOINTMENT
WEAR A MASK
Again, we require masks be worn in our building and throughout your visit.
SOCIAL DISTANCE
To the best of the staffs’ ability, patients are kept at a distance during their therapies. If you have any specific concerns or require additional precautions, we’d ask that you inform us before you arrive.
AFTER YOUR APPOINTMENT
SANITIZING
